eDoc4u will use reasonable measures to protect the personally identifiable information you provide through the Service (Your Information) from loss, misuse, and unauthorized access, alteration, and destruction.
eDoc4u has provided this notice to describe the online information collection and use practices in connection with the Service. This privacy policy applies only to the information collected directly for the Service, and does not apply to any other information that eDoc4u collects about you on other eDoc4u owned, operated, or sponsored web sites, or in other forums, including in e-mail correspondences or over the telephone.
eDoc4u may use Your Information to create non-identifiable summary information or aggregated data, and may share this non-identifiable summary or aggregated data with your employer or health plan. Such information will also be used to help improve the quality of the Service. If required for tax reporting, eDoc4u may also share with your employer or health plan information relating to any cash or non-cash incentives received by you. Other than as stated in this policy, eDoc4u will not disclose, give, sell, rent, or loan any of Your Information to any other party, unless legally required to do so or unless you specifically direct or authorize us to do so. In some circumstances, eDoc4u's use of your information will also be subject to the requirements of the Health Information Portability and Accountability Act (frequently referred to as HIPAA). This may be the case when you submit information to eDoc4u when it is acting on behalf of your health plan (e.g., when you complete a Preventive Care Assessment online using the Service as a health program offered by your health plan), in which case your health plan's HIPAA Notice of Privacy Practices will apply. Any uses and disclosures of your personal information described in this policy will then be further limited to only those uses and disclosures also stated in your plans HIPAA Notice of Privacy Practices. Health care providers and other persons whom you authorize to access your eDoc4u records will have access to portions of Your Information. Their access will be limited based on the choices you make in your authorizations. If you select "Email Recommendations" or "Email Personal Health Record", all applicable content available to you online, including Your Information, if applicable, will be emailed to the email address you have entered in your profile in unencrypted form.
Your personalized results will be available to you after you've completed the survey. An agent of eDoc4u hosts the registration page and the entire Service. The Service analyzes your responses, and that information is stored on the servers owned by the hosting company. The hosting company has agreed to maintain the privacy and security of Your Information and to return all of your responses to eDoc4u. Please be aware that it is possible that technical staff at the hosting company will see your answers while helping eDoc4u solve technical problems.
eDoc4u reserves the right to modify, amend or restate the Privacy Policy at any time and to post any changes to the membership information area. It remains your responsibility, as a participant, to periodically review the Privacy Policy and to contact eDoc4u within seven business days after any changes to this policy if you do not accept any amendments. Your failure to notify eDoc4u that you do not accept any changes in the Privacy Policy made by eDoc4u means that you accept the changes and that the amended terms will apply to you. This version of the privacy policy is effective as of 2/25/15. Please note that if your information is also protected by HIPAA and so also subject to your health plans Notice of Privacy Practices, then the more restrictive of this policy and that Notice will apply to describe what uses and disclosures we may make of any personally identifiable information you provide to us.
The Service contains links to other Web sites not operated by eDoc4u. These links are provided as a convenience and do not constitute an affiliation with, endorsement or sponsorship of the third parties that operate those Web sites. eDoc4u is not responsible for the availability or reliability of the content of these third party Web sites. Any information you provide to those third parties is not subject to the terms of this privacy policy, and eDoc4u is not responsible for the treatment of the information you provide to those third parties. We recommend that you review the privacy policy of that third party, and of any third party to whom you provide personally identifiable information online.

This document describes the terms and conditions applicable to a participant (you) in the use of this service (the Service) offered by eDoc4U and its affiliates, including Conduit Corporation (Conduit). By registering for the Service and submitting your health information, you agree to be bound by all of the following terms and conditions:
The recommendations and health proposals provided by the Service and eDoc4U's hosting site (the Site) are for informational purposes only. As a participant in the Service, you assume full responsibility and risk for the appropriate application and use of your health information and for following any recommendations or accepting any health offers provided by the Service.
No information provided by the Service is intended to diagnose, treat, or follow up on any medical disease or condition, nor is it intended to replace an actual "hands-on" consultation with your qualified health care provider. REMEMBER, THE SERVICE IS NOT A SUBSTITUTE FOR A COMPLETE MEDICAL EXAMINATION BY YOUR DOCTOR! ALWAYS CHECK WITH YOUR HEALTH CARE PROVIDER REGARDING ANY QUESTIONS OR CONCERNS THAT YOU HAVE REGARDING YOUR HEALTH. The Service does not create a doctor-patient relationship with you but simply strives to improve the doctor-patient relationships you have with your doctor or other health care providers by providing you with relevant medical information. eDoc4U assumes no liability whatsoever for the information provided or for any diagnosis or treatment made in reliance thereon.
The information available at the Site and provided through the Service is protected by applicable copyright and patent laws. As a participant, you may not reproduce, retransmit, distribute, disseminate, sell, publish, broadcast, or circulate any such information provided to you. This includes, but is not limited to, translation into foreign languages, storage on CD-ROM's, and any other use. You may, however, display, reformat, and print any information for your own personal, non-commercial purposes, and you may use your own health information for any purpose.
Due to the unpredictable nature of electronic transmission and distribution, there may be inaccuracies, omissions, and typographical errors in the provided information which are unknown to eDoc4u. We are committed to providing accurate information; however, the information may incorporate expert opinions and experienced medical judgments that eDoc4u has not developed and for which eDoc4u cannot be responsible.eDoc4U cannot warrant the accuracy of information or suggestions provided by the Service.
eDoc4U has implemented administrative, physical, and technical safeguards to reasonably and appropriately protect the confidentiality, integrity and availability of any Protected Health Information as defined and regulated by HIPAA, HITECH, the Omnibus Rules, applicable state laws and any other related law, rules and regulations currently in effect or effective in the future (“PHI”), to which you have access, and to prevent the use or disclosure of Protected Health Information except as allowed by law or as agreed by you. eDoc4U delivers and maintains in compliance and will continue to be compliant with all such laws related to PHI.
Neither eDoc4U nor its parents, subsidiaries, affiliates, agents, licensees or service providers or their respective officers, directors, employees, contractors or assigns shall have any liability to you arising out the Service due to any cause not within its reasonable control, such as an act of God, war, riots or civil disturbances, strikes, accident, fire, transportation conditions, labor and/or material shortages, governmental controls, permits, embargoes or the negligence or wrongdoing of any party other than EDoc4U or its affiliates
You are responsible for safeguarding your password. All access to the Service and the Site using your password, and any data modifications to your records in connection with such access will be your responsibility and presumed to have been executed with your consent. eDoc4U reserves the right to modify, amend, restate, and/or terminate any or all of these Terms and Conditions at any time and to post any changes to the membership information area. It remains your responsibility, as a participant, to periodically review the membership information area. If you do not access the Service for a period of two years from registering for the Service, eDoc4u may archive or purge your information.
If any provision of these Terms and Conditions is deemed invalid, then the remaining provisions will continue in full force and effect. These Terms and Conditions, all intellectual property issues, and your rights and obligations shall be governed by the laws of the United States of America and the State of Tennessee. Any dispute you may have with eDoc4U or any of its parents, subsidiaries, affiliates, agents, licensees or service providers, or their respective officers, directors, employees, agents, contractors and assigns shall be resolved exclusively in the courts of Davidson County, Tennessee.
In order to use this Service, you must (i) be at least 18 years of age; and (ii) acknowledge that it is solely your responsibility to follow up with the appropriate medical provider for any medical services you think you need as a result of participating in the Service. It is your responsibility to review the Terms and Conditions from time to time to determine if any changes have been made.
